How to export sql table into excel sheet in required format.

Hi,

Exporting the sql datas into excel sheet as required format means in excel sheet first two rows should be empty, then in
next row one line text and one line gap then again the tables from next line from should start from second row.
The xl sheet should be produced like this format.
here i attach format of xl sheet.
