Storing Additional User Information: "Create UserWizard Control" with additional fields (including TextBox, CheckBoxList and DropDownList controls) in 1 single step.

Hi everybody,
I really hope someone could help me with this.
I am trying hard to customize my CreateUserWizard by adding some additional fields that should populate a brand new table called UserProfiles. In order to create this brand new table I followed all the instructions provided in the following tutorial with some additions:

Basically the additional user info I would like to include in this new table are the FirstName, LastName, Country of birth, Date of Birth and the Gender.
This is the code for the New Use Wizard:
