Webform to save database connection and later use

 
I wanted to to build a simple form where user provides Database Connection information

Host, DB Type: SQL/Oracle,Port,Authentication Mode,User Name,Password,etc into the WEB FORM and saves it into pre configured database table (defined in web.config)  
User later uses one these saved connection to run Database Queries.
I am able to Add and List database connections records but not sure how to use it later in the form
