How to input data from textboxes and listboxes to sql database in c#?

Hi,
 
I have been trying to fiqure this out, but cannot seem to put the pieces together.
 
I have a work order form with textboxes and listboxes. The customer information would be inputed in the textboxes. The work order options will be in listboxs. The selected listbox item will be inputed in the database.
 
After it is filled out, there is a Enter button.
