Create controls depending on paramaters in Stored proc

Hi everyone!
I've created a stored procedure to find all the stored proc's in the database and what paramater name and paramater type the stored proc takes. I've created a method that populates the controls manually when a report is chosen. The problem that I'm facing is that it is populating only one control for the datatype ex. the dateTime is creating a control only once where it is supposed to create 2 dateTimePickers. Can anybody tell me what I'm doing wrong please?
(when choosing the report in the dropdownlist it automatically gets the stored proc so there is no need to instantiate it). The reason that I'm doing this is because I'm using only one form to give several reports. This is what I've done:
