Form Button Click submit duplicate database records

 Hi,
 I have a simple input form with an sql data source control and a submit button.  On click of the button duplicate (4 records) are inserted in the database.  It should only be on record inserted per button click.  Would anyone tell me what I'm doing from by viewing the below code and point me in the right direction.
