File Upload

I have this page which has a file upload box . I have a button on the page which uses to javascript to add another file upload box. All of the file upload part works perfectfly. I am trying to add a piece to the webpage that inserts a entry into a database for every uploaded file.  I want the database entry to contain the full path to the file including the filename (dont worry about security this web app is for internal IS users only). Everything is working except that the filename that shows up in the database is that of the last file in the series of uploaded for all the entries.
