Submitting form to Sql only submits my script for listbox

Hi, on submitting the form to sql i only get whats selected in my listbox. There is a script that tells each selection in the listbox to populate a table in sql, however that is my only button on the form and there are other fields that need to be sent to the DB when pressing submit. Do i possibly have the button in the wrong place? Thanks!
