Unexpected behavior w/ dynamically loaded user control

Hi,
In my aspx page, I have a PlaceHolder control. Based on input parameters received by the page, I want to dynamically load a user control into the PlaceHolder control. Up to this point, everything is working and user control is loading fine.
However, when the user makes entries into the user control, it's disappearing. The particular user control I load is used for accepting or declining a task and has a simple RadioButtonList with Accept or Decline options. If user selects decline, it then requires the reason for declining the task. Here's what the code looksl like:
