Saving an integer value from 2 forms to 1 table

I have 2 user controls, one is called salon-notes.ascx and one is called customer-service-notes.ascx. I have a aspx page called Salon.aspx where there are 2 tabs called Salon Notes and Customer Notes. When the user submits data in the Salon Notes tab I want it to save in the Notes table with a value of 0 (The array class is shown below) and when the user submits data in the Customer Notes tab I want it to save in the Notes table as well with the value of 1. How do I do this? My code and database table is below. Please help.
