WSE 3.0 MTOM Example Request

I'm trying to create a WebService function using WSE 3.0 and MTOM to upload a file to a web service. I keep reading articles on how this can be done, but I'm just not finding an example. I learn best by example...

Can anyone provide an example, or link to an example, of how I can setup the Web Service to accept an MTOM message which includes a file (any type file)? If possible, I'd like to see a complete example, from Javascript calling the Web Service with the file attachement, to how the Web Service function handles the file attachment (preferably the file attachment being saved to a database, but not necessary... I think I can figure that part out...)
