Build an INSERT statement based on form data

I hope this isn't too basic but I am new to ASP. I have a form that contains a bunch of Yes/No checkboxes and a few text box fields. Ultimately I want to create a database record (SQL Server) based on the answers to the Yes/No questions along with the text fields being the Customer information part of the database record. What is the best way for me to build a query and execute the INSERT statement?
Is it as simple as a long script that evaluates the checkbox.Text value in a bunch of IF statements and piece together the INSERT statement? If so, how do apply a connection string and eventually execute the INSERT? Is the best way with an OnClick event?
Mainly looking for design ideas and help with the back-end work. Thank you.
