Web Service "is not defined" error

 I've looked for an answer on this pretty extensively.
What I've checked:
.asmx file is referenced in the right location with ScriptManager
Web.config has the correct entries for web services
Correct namespace is used
Not using namespace for the service
[ScriptService] tag exists

I'm hoping I missed something here.
I am using .NET 2.0 with AJAX Extensions 1.0 in Visual Studio 2005.
Error is "QCReporting is not defined." When the web service is called.

Any help is greatly appreciated! Code follows.
