Dynamically creating controls

Hi All,

I have a 3 text boxes, one image , one dropdown field and then a Add button. On clicking this add button I want all the above 5 fields to appear again, i.e. new fields created dynamically. This process will go on for the N number of clicks user wants, and then finally save all these on a click of save button. This is a web application.

Please suggest some way out to achieve this.
