Need help with Custom Data Types

I'm using Linq to iterate though a list which produces the below data
Scores (count = 6)
(0)    2
(1)    2
(2)    4
(3)    2
(4)    3
(5)    4

What's happening is I have 6 Risk Categories.  Each Risk Category has 5 Risk Choices that can be picked.  The above data is the result of one of those rows.  The nunber represent the Risk Choice that was picked for each Risk Category.
What i need to do now is Produce a Chart with Risk Choices, per Risk Category..
So basically, (riskFactor, Choice1, choice2, etc.)
I'm using the below but I seem to be going around in circles.  Any help would be appreciated.
