help me with using text box and submit information button

i know its an easy thing but still need help with
i want to make a form like this on my website in which information will be filled by the visitor of my website
Name : [TEXTBOX]
E-MAIL: [TEXTBOX]
Product 1:{CheckBox}                    Product2:{CheckBox}


[SubmitButton]
Now how the filled information will go in the data base and how clicking on submit button will save the filled data in database
please explain me step by step how to set up database , how to make table, and how this information could be saved
please dont give me any links cuz its difficult to understand , if you have some time then please explain me step by step
