Multiple Entry

Dear All,

I have to make a input screen like this.

Parameter Name - Value Enterd

Parameter1 - Yes/No/NA Drop Down

Now there is 30 parameter where by default NA will come in Value and when user will press submit all the 30 rows will be saved in database.

Please give me concept for this and if you have any example for this it will be a great help.
