How to generate Checkboxes at runtime from datasource

My requirement is show multiple checkboxes in different lines on the panel.
i have one table i.e. tblProduct with columns product_ID and product_Name
Here I want to take all the rows from the above table and generate checkboxes for them
like
Product_ID (1) - Product_name(1)
(2) - Product_2 
(3) - Product_3 
(4) - Product_4 
(5) - Product_5
For this I want
1) How to bind table to generate multiple checkboxes
2) Genreate them on different rows
3) How to assign unique ID
4) Concatinate the Product_ID - Product_Name
