SQL Command Passing Null Value from asp.net page

I hope someone can help, i've spent more then two days trying to get this to work. I have re-worked  the code many times. I have a aspx page the user enters info, i create the sql connection, command object, add parameters, execute, a recrod gets added to the sql table but all the values are null. I have the fields set to null for the sake of troubleshooting. I have verified that I can get at the values I don't understand why thye are not getting assigned to the parameters and passed to SQL Server.
