How to Concatenate a string for Insert statement

I'm trying to create a string to perform an insert using a table adapter but I'm having issues with getting the syntax correct.  I also think there might be a better/cleaner way to do this but don't know. After I get the syntax correct to insert a record I need to return the scope identity. I think I have it set up correctly but not sure. Thanks for any ideas. Here is the code behind I'm trying to get right:
