Insert date with ammended number to sql

Hi,
I have a simple form, it gets the date and allows a user to input a 6 digit number.
The user then clicks a button, the page needs to take the date and apend the 6 digit number and insert it into a SQL database.
So:
 1/26/2010
Text Input is : 123456
 
It appends both, so it would get:    1/26/2010-123456
Then with the button click, it inserts that data into a field in a table.
Any help with the code or ideas would be great !
