displaying form filed values in email body

Hi,
 I am new this forum. If this post needs to be moved to another thread, please let me know.
I am new to programming. I started learning Visual Studio Web Developer 2008 and doing a small project about Telephone number mapping to customers etc...this site is really helpful & i learned how to add data to sqlexpress using basic features in web developer tool.
I was able to add new entires to my database & I am also getting an email confirmation but, not getting all the fileds in the body like. I have CustomerName, PhoneNumber, Email Address..(almost 8 fields that i am adding using the form). I want to see all the fields that i entered in my email. How do i do that. below is my code:
