Need real help to resolve this simple code

Hi,

I am trying to write a prototype that will enable one to pass a parameter from a form to a stored procedure. This stored procedure provides a result set that I am trying to export to excel. I have previsously successully done this with simple sql statement. Now I have modified the code (which uses data reader) to accomplish this. This is a single file code. I am not sure why I am getting the following errors:

Keyword is not valid as an identifier

Filestream not defined

Streamwriter not defined
