Adding a set of textboxes to the page

Hi,

I am making a sort of contact form, but i'd like it to be dyanmic in that the user can press a button and more text boxes will show (or disappear if press another).

So i start out with a page with a line of 5 text boxes. These will be all sent to an email address on submit.

I'd like to have a button which will add another set of 5 textboxes, which will also be sent to an email address.

 How can I acheive this? I am thinking i'd need to make a 'group' of textboxes (the 5) and with the button add a new group?

 

Then it will be complicated because the submit button would need to email this new group too.

 

Also, how can I format the information that is being sent via email? e.g. I'd like the textbox values to be layed out a certain way in the email they are sent via...?
