How i could add dynamic fields form like this http://accelerato.rs/azure/apply

How i could build dyanic field form like this one http://accelerato.rs/azure/apply
for example if i have form with Name , address ,city ,date of birth, email extboxes
and button Add new
when user fill the form and press add new then new form will be added after the first form with the same fields?
