how to set due date for multiple month

Dear All forum member,
I have requirement that I want to set due date for payment collection for each month. 
Let me describe my requirement in brief.
In my  current application my client wants that He wants set 
starting and closing date for each month for payment collection . 
Now I have decide to have two text box for starting data and closing date.
And after that I want to have multiple check box for selecting month. After selecting month when user click on save button the mentioned staring date and closing date will be applicable to selected month. 
Now my problem is that how will do it and what should be my table format so that at time of collection I could check that payment date for the month should be in between starting date and closing date.
