I want to save the user input and want it to display in MS Word as this:

Now, at the bottom of the screen there will be a button that generates a word document. This is a simple example.
I want the next user in the same session to reuse the form. So, Mary would go in and answer the 2 questions and John would go in and answer the two questions
After they click the No button for the last question both info from both users will appear in the MS doc after generated so the doc would look so ...
