unsure what to write in connection string

Hello, i have never use SQL. I usually use Access as it's easy to write the conncetion string. but I am unsure on what to write on each specific value of the string
for the data source, how to know my server address or does it refer to my computer name and for initial catalog, do i just write the name of the file or the full path directed to the file?
 
Data Source=myServerAddress;Initial Catalog=myDataBase;User Id=myUsername;Password=myPassword;
