Adding a comment box to a dynamic page.

Hi all,
I have a dynamic "Concert page" which shows a specific concert at a time,( performers, date and time, pics etc').
It all works fine each time the page loads it is updated with the required data from the DB.
I would like to add a comment box to each concert, something like this free one:
http://www.htmlcommentbox.com/
My problem is: how do I make (and show) a different comment box for each Concert.
Hope I was clear enough.
Thanks.
