I am trying to use the example at http://aspnet.4guysfromrolla.com/articles/070506-1.aspx to add two custom fields to the CreateUserWizard, FirstName and Company. Everything is set up and appears to be working fine, except the added fields are not getting saved to the new database table I created.

The only differences from my page and the example are I am using SQL Server 2000, not SQL Express like they are and I am not dividing the account creation form into steps. I just have it on one page. Below is the code I am using. Anyone have any idea where I am going wrong?
<asp:CreateUserWizard ID="CreateUserWizard1" runat="server" ContinueDestinationPageUrl="~/account/Default.aspx">
<WizardSteps>
<asp:CreateUserWizardStep ID="CreateUserWizardStep1" runat= ...
