Uploading a photo and submitting form data

I have a database that I would like users to be able to submit data to.  My website is photo sharing site and I also want users to be able to upload a photo.  I'm using a formview for users to submit the information (for the photo) to the database.  I then use a FileUpload tool with the following background script, unfortunately when I run the site it tells me that the FileUpload1 does not exist.
