Create a reusable contact form class

Hi,
recently I have been ventured into creating reusable classes and controls and I want to create a reusable contact form class that will automaticallygenerate the required textbox fields etc
I want to go from the code below and turn it into a control / class
My question is how to dynamically create the text box elements and how to allow me to send an array of elements e.g. (name, emailaddress, address, telephone number, ) into my control so that it knows to then create a text box for each.
