design a questionnaire

Hi All,
My company asked me to design a questionnaire site. I have few issues which really are roadblocks for me.

1). How do I control the time? The whole questions will be answered within a time frame.
2). Every time page shows one question with answer choices. How do I show how much time left for the whole questionnaire? and at the same time a digital clock on the page showing the time is ticking.
