How to submit record in database?

 Hi Everyone,
 
I have these checkboxes on my web page. I have this code with me which is supposed to fill in the values 'Y' or 'F' in the database under the given columns. I don't know whats wrong with this code but the values are not getting in the database. Please let me know if you have questions. Any kind of help will be appreciated.
