Please any one help me in writing the code in this
I have 3 fileds
1)empId-->textbox
2)Roles-->4 radiobuttons(MN,PL,TL,CL)
3)Responsibilities--->3 check boxes(profile,register,change password)
I have 3 tables
table1:

empId roleId resId
the form conatains the empId,Roles(radiobuttons),Responsibilies(chechboxes)
for example I have to enter empId=1
select one radiobutton that is "PL"
