Unpivot and tranfer data simultaneously

I created Store Procedure and get column in two table.  Here is the data table that I've taken:

I want insert column DebitAmount và CreditAmount, Entry Line.
Column DebitACC và CreditACC into one column AccountNumber.  as the image below.
