webformmailer.asp - Uploading Files with a java Script

Hi there
Im having some troubles uploading files to my webform. I have tried many solutions suggested on the internet, but I cant seem to get it to work. Im using Godaddy and webformmailer.asp. Please help.
Problem: I cant seem to get the files to upload, and send to email as attachments
