Geting the values from a dynamiclly populated form

I am building a website use asp.net C#.  Now depending on who is logged in I need to be able to populate a form based on the user’s database.  Since this will be different for each user, I am using a listview to populate the fields of the form, mainly checkboxes.  Now from code behind how to a grab those values if the checkbox’s are checked.
 
So let’s see if I can clarify this better.  The database has a list of options which will populate a form of check boxes.  If the user’s needs those options then they can select which ones they need when they click finish I need to be able to grab the value of the checkboxes and place them in another database, but I am not sure how to get that da ...
