Data Insertion / Record Creation v ID field (primary key)

Web form below opens without error in browser, but does not insert data enetered into fields into database, creating new records; when the "Add" button is clicked, the data remains in the fields as entered; new records do not appear in the DataList at bottom nor in the actual database when checked. There are four (4) text fields - as evidenced in the picture and code below - and one (1) ID field that is neither pictured or referenced in the code.  The ID field is set in the database to integer, as the primary key, and autoincrement from 1 and by 1. I suspect that the problem lies in that something more needs to be done with the ID field somewhere - code or database - that will allow records to be created.
