Custom user membership and authentication

I am developing an ASP.NET website which requires that the use of a custom use creation and authentication. In this case, unlike many other developers, I am not allowed to use controls like the login controls and the such to create the user since my boss thinks it gives less flexibility to the website. I was wondering if someone knows a good website/blog which provide a good tutorial to do this. Specifically I will need to: (1) Create the database for the user, (2) Save user authentication (password, email, username) in the database, (3) allow user to change his password, and (4) give the user a new password if forgotten.
