Rollover Image Button Problem

I used a tutorial online to try and make the image buttons have the rollover effect.

I did as the tutorial instructed and when I finished and ran the code, instead of the images coming up for my buttons the words:: Submit Query :: showed up.  I'm not sure what in the coding would have caused that.
